Our home city

Hospice care in the city of San Bernardino

San Bernardino is our home city. Beta Hospice has one office, on North D Street, and it is the only office we have anywhere. Care does not happen there. Nurses, hospice aides, social workers and chaplains come to the person's house, apartment, board-and-care home or nursing facility in any part of the city. Being based here means our team knows the streets, the hospitals and the help lines that San Bernardino families use.

Neighborhoods and nearby communities we serve

San Bernardino is a large city. These are the areas families ask about most, and all of them are a short drive from our office.

Hospitals and care facilities nearby

Two Dignity Health hospitals serve the city: St. Bernardine Medical Center on North Waterman Avenue and Community Hospital of San Bernardino on Medical Center Drive. Arrowhead Regional Medical Center, the county hospital in Colton, is a few miles south of downtown. Many San Bernardino families first hear the word hospice from a doctor or case manager at one of these three hospitals.

Hospice care continues at home after a hospital discharge. When a hospital team raises hospice, we work with the discharge planners so that the first nurse visit, the hospital bed and the medicines are arranged around the day the person goes home. You do not have to set this up yourself. Read where care happens.

We care for people wherever they call home, including nursing facilities, RCFEs (residential care facilities for the elderly) and board-and-care homes, in coordination with facility staff. San Bernardino has many small board-and-care homes in residential neighborhoods, and a hospice nurse visits a person in one of them the same way the nurse visits a private house.

Medi-Cal plans in this city

Most people with Medi-Cal in San Bernardino County belong to a managed care plan. The county's Medi-Cal plans are Inland Empire Health Plan (IEHP) and Molina Healthcare, plus Kaiser Permanente for people who meet Kaiser's rules for joining. Medi-Cal covers hospice, and when you are in a managed care plan, hospice is arranged through that plan.

These are the county's plans. Listing them here does not mean Beta Hospice has a contract with each one. Whether we are contracted with your plan is checked for each patient. Call us at (844) 646-4434 with your plan card and we will tell you where things stand before care starts.

If you have Medicare, the rules are the same in every city. Medicare covers hospice care related to the terminal illness. Under Medicare, most people pay nothing for hospice care itself. There can be a copay of up to $5 per prescription for symptom medicines and 5% of the Medicare amount for inpatient respite care. Medicare does not pay room and board where you live. Read paying for care.

Community resources

These are public services, not part of Beta Hospice. They can help with needs hospice does not cover.

  • Perris Hill Senior Center, 780 East 21st Street. One of two senior centers run by the City of San Bernardino, with meals, classes and social programs for older adults.
  • Fifth Street Senior Center, 600 West Fifth Street. The city's second senior center, near downtown.
  • 211 San Bernardino County. Dial 2-1-1 from any phone in the county for referrals to food, housing, transportation, caregiver and utility help. Open all day, every day.
  • Senior Information and Assistance, 1-800-510-2020. The line for the county's Department of Aging and Adult Services, which is the Area Agency on Aging. It connects older adults and caregivers with meals, caregiver support and Medicare counseling (HICAP).

Spanish-speaking families

Many San Bernardino families speak Spanish at home. Our team includes Spanish speakers, and you can ask for Spanish when you call. Read about hospice care in Spanish, or switch this page to Spanish using the language link at the top.

San Bernardino hospice questions

Does Beta Hospice come to homes in San Bernardino?

Yes. The city of San Bernardino is where our office is, and every neighborhood in the city is in our service area. The nurse, aide, social worker and chaplain come to the person's home. Nobody has to travel to us.

How fast can a nurse visit in San Bernardino?

A hospice nurse answers the phone 24 hours a day, 7 days a week, and can visit when needed. For a new referral, our standard is a response within two hours, 24 hours a day, 7 days a week. Once a person is on service, visits are scheduled in the plan of care, and the nurse on call decides when an extra visit is needed.

Can hospice come to a board-and-care home in San Bernardino?

Yes. We care for people wherever they call home, including nursing facilities, RCFEs and board-and-care homes, in coordination with facility staff. The person or their representative chooses the hospice, not the home.

Where is your office?

Our office is on North D Street in the city of San Bernardino. It is our only office. The address, hours and a map are on the contact page.

Sources

These sources support the general information on this page. For one person’s care and coverage, talk with the hospice team.

You do not have to decide alone.

Call (844) 646-4434 any time, or leave your number and someone from Beta's hospice team will call you. No decision is needed to call. Most families start with a question.

Only a hospice doctor, together with the person’s own doctor, can confirm hospice eligibility. A website cannot.

  • A nurse answers 24 hours a day, 7 days a week
  • You can stop hospice at any time
  • You have the right to choose your hospice

If this is an emergency, call 911.

Fields marked with a star are required.

Please do not include medical details here. We will ask by phone. This form does not enroll anyone in hospice. Choosing hospice is always the patient's decision.
