What happens next

We call you back. A nurse confirms the details and any urgency.
We contact the patient or family. With their agreement, we arrange an informational visit. Only licensed hospice staff discuss electing hospice.
Evaluation. Our medical director and the attending physician determine eligibility.
Admission and logistics. We supply transport from the hospital to home, together with the equipment, supplies and medicines related to the terminal illness; in-stock items are delivered within two hours. We verify orders and destination, coordinate delivery and transport, confirm the first visit and explain how to reach us.
