Notice of nondiscrimination
Beta Hospice Care, Inc. complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, sex, age or disability. We do not exclude people or treat them less favorably for any of these reasons. This notice explains the help we provide so everyone can communicate with us, how to file a grievance with us, and how to file a civil rights complaint with the U.S. Department of Health and Human Services.
Our commitment
Beta Hospice Care, Inc. complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age or disability. This notice is given under Section 1557 of the Affordable Care Act and its rules at 45 CFR Part 92.
Sex includes pregnancy and related conditions, sexual orientation, gender identity and sex characteristics, including intersex traits. Under California law, including the Unruh Civil Rights Act and Government Code section 11135, we also do not discriminate on the basis of ancestry, religion, marital status, medical condition, genetic information, citizenship, primary language, immigration status, or military or veteran status.
Help we provide so you can communicate with us
We provide the following free of charge and in a timely manner, when needed to give a person an equal chance to take part in our care.
- For people with disabilities: reasonable modifications, and appropriate auxiliary aids and services, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats).
- For people whose primary language is not English: language assistance services, such as qualified interpreters and information written in other languages.

How to file a grievance with Beta Hospice
If you believe we failed to provide these services or discriminated in another way on a basis listed above, you can file a grievance.

File in the way that works for you
You can file in person, by mail, by fax or by email. If you need help filing, the civil rights coordinator is available to help you.
- Step 3
We investigate and answer
We acknowledge your grievance within 5 business days, investigate it, and send you a written decision within 30 days of receiving it. If you disagree with the decision, you may ask the administrator to review it within 15 days, and you may file with the federal Office for Civil Rights at any time. No one may retaliate against you for filing a grievance or taking part in an investigation.
How to file a complaint with the federal Office for Civil Rights
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You do not have to file a grievance with us first.

Where this notice is available
This notice is posted on this page and at our office, and is given on request. It is available in large print and other formats on request. It is posted at the reception area of our San Bernardino office and included in the admission packet every patient receives.
